Hello, I was informed that applying ice is most effective within the first 48 hours of an initial injury. After that, heat is better because it increases cirlculation and therefore, a faster healing rate. My grandmother just had her knee operated on 2 months ago and she is still getting ice packs by her PT. Is this right? Shouldnt she be getting heat on that knee? Aside from this, she says she isn't even getting any ultrasound treatments! Can anyone tell me what her PT might be trying to do? Or maybe the rationale behind his treatment (or lack there of)? Thanks...

Hello PTstud, It depends on when they are using the ice. I heat TKA's first, and ice them afterwards, often in the position they most lack (extreme flexion or extension).
I never use US on a TKA as there is still a debate as to whether the plastic or the methylmethacrylate cement would or could be affected. Risk vs. reward indicates that the benefits from US probably would not outweigh damage to the implant. Metal, however, is a homogeneous substance and transmits US energy.
You should purchase a book from Kenneth L. Knight called "Cryotherapy in Sport Injury Management" This book is OUTSTANDING, and the cryokinetics program inside has allowed me to return athletes to sport significantly faster than any other program I have seen!
