whats the best modality to use for incisions? tens, ice, ultrasound? any suggestions?

What is the problem in the incison are - pain or immobility.  If it is pain and inflammation then ice and estim may be of some benefit.

If immobility is the issue then I see no point to use modalities as your best approach would be scar massage in the clinic and then teach patient how to perform at home.

IMO scar tissue is simply superficial fascia and responds very well to myofascial type work along the incision followed by exercise to stress the incision along the direction you want to increase mobility.
