I have a question on whether providing gift certificates to Office Managers at referring groups would be considered a form of kickback? It seems all the literature states that kickbacks involves the Doctors' directly.

As long as you adhere by the guidelines set forth, you can do this. There are financial limitations of $50 per month and $300 annual total for each physician group and this includes all employees there. Cash is very much frowned upon and gift certificates are basically a cash substitute (gray area). Legal loophole exists however. Pay for this out of your personal account and expense back to your clinic. Do not pay for this out of your clinic's operating or admin account.
