Sum of n responses/ n, -1, then multiply by 25
N- #of completed responses. Cannot calculate if >3missing it's

Thanks but i need to be able to give the form to the patient to fill out in the lobby. The website does not help

Just use the formula I gave on the bottom of the post, check the score with what you get from the website. Should work, if not, it should be easy to find the directions thru google

The formula for calculating the score is on the bottom of the form available at the website given above. I always make sure to remind the patient to fill it out based on performing the task regardless of what UE they use to perform the task.

I use the DASH or QuickDASH quite frequently and find them very helpful.
