I have a patient who had cancer 20 years ago with shoulder pain after a box fell onto her shoulder. She was very tender along her upper traps, so I used E-stim on her. She told me another therapist had told her estim causes cancer, which I never learned in school, I told her that ultrasound as a modality would be contraindicated.

I have searched for articles concerning contraindications to using TENS units, but have not found any. All the research says is to not use over a carotid sinus, or people with cardiac pacemakers.

Does anyone have any additional info on this?

Cindy, I think this is a perfect example of the patient hearing what they want to hear not what was really said. There are no known contraindications for use of TENS for pain in clients with cancer. I have used it frequently to help control their pain. At some point, it is not effective though. In my experience, the higher the dose of their pain meds the less effective TENS becomes. Good luck and be confident in your knowledge.

Thankyou FallsPT! You just about hit the nail on the head with this patient. What has complicated things further was that at our last treatment session she found a lump in her breast. I did not use TENS that day on her for the mere fact that she had so much anxiety about the lump and her MD told her not to use TENS anymore for her shoulder.

Hi,

There is no real contraindications for cancer but more a consensus about their use. By precaution, it is not really a good idea to use TENS when lymphatic system is involved or metastastis are present.
BTW, in these unfortunate cases TENS is useless.

Henry,
In cases where there is known cancer, and it is painful and not treatable, TENS actually can be rather helpful in improving quality of life for the time they have left. Ive had MD orders for it when they know that things look bleak. Anecdotal and empirical only, no studies to cite for y'all.

I have heard of cases where TENS was used to help relieve the pain caused by cancer from colleagues who knew people who worked at Sloan Kettering Medical Center in NY (known widely for their specialization treating patients with various forms of cancer).

Hi,

I worked 3 years for a TENS company and we used many modalities for such pain without success. Often you have a transcient relief of one hour/day but pain becomes worse in tme.

I prefer now neuromobs and breathing technique.
