Hi Sarah! I've never had this type of injury before, but I may be able to offer you a bit of info. There are several surgical procedures that can be performed with this type of injury, but one of the most common is deepening of the peroneal groove, just under the outside of your ankle. With this procedure, the surgeon creates a larger groove and this is turn will 'house' the tendons more efficiently.
Quite a few surgeons tend to try a non-operative approach first by reducing the inflammation of the tendons. They will usually place you in a below-the-knee cast for about 6 weeks and provide you with some anti-inflammatory meds. However, if you are very active, i.e. an athlete, the chances of subluxing again are pretty high.
Hope this helps!

You might want to hit the library and dig up this article:
TRAUMATIC PERONEAL TENDON INSTABILITY. The American Journal of Sports Medicine, Vol. 24, NO. 5, September/October 1996.
The general concensus is that surgery is the requirement, except in certain instances. The more you snap that tendon over the bone, the more damage you can incur.

Hey Sarah,
I have actually had a pretty severe peroneal subluxation when I was in college. I played soccer at a D1 school, so I had access to the very best care. It happened when I went to kick the ball and caught the toe of my shoe on the ground and I then fell forward. I was very lucky that they caught it very early, I was in a cast for 6 weeks NWB, and then a walking boot for another 6 weeks progressing to FWB. The goal was to allow scar tissue to form to keep the tendons in place. I then started aggressive rehab. When I returned the trainers taped my ankle in a special way to hold the tendons down. Today, I only have problems when I run on very hard surfaces, or play indoor soccer.
If you interested look up what Curt Schilling went through in order to play in the world series.
