Hi, I am networking for those with experience dealing with complications in prosthetic training. Specifically, I need advice with how to prevent fluid blisters from developing at the old wound site once weight bearing is resumed. This BKA is five years post and alternative prosthetics have been trialed that take the weight off the end of the stump, yet the site remains vulnerable to fluid build-up. Wound care staff list pressure and shear as the cause, but I'm suspecting something else. Could this be a proprioceptive response? What have you found to be successful P.T. in assisting the transition back to weight bearing. This pt. has been nonweight-bearing and restricted from donning the prosthesis for six months due to wound care precautions.

I have somewhat limited prosthetics experience, but here goes...
I would agrtee with the wound care staff that pressure and shear are the culprits. I know that is the accepted mechanism for fluid blisters, and I would think that managing that would be helpful.
That may consist of different socks or even using thin felt or "moleskin" to unload the blister area...???
Being NWB that long might translate to a longer progressive workup until able to be FWB...
