Well don't feel bad because it sounds like the surgery wasn't done properly as the implant was probably a little bit too large for the space, which didn't become a problem until some age-related arthritis moved into the joint as well. So don't focus on the past because you didn't cause his elbow pain at all. You can try modalities to the joint including heat, ice, electrical stimulation, cold laser, cross friction, deep sports massage, joint mobilizations, and passive range of motion. If none of these things work then you can progress to light strengthening therex, but the only cure may be to do an arthroscopic surgery to clean out the osteoarthritis and either revise or take out the implant.
