You may want to use the Guide to PT Practice. In it, there is a pretty thorough outpatient intake. You could use that as your template and add or subtract things that fits you best. In the second edition, page 699 is where they start. I just recently updated ours and found that I wanted to add more to the symptoms within the last year section. Also, you may want to (if not already) put in a question regarding falls within the last year, per Medicare requirements. Let me know if you have any trouble finding the reference.
