Hi Everyone, I work with very young soldiers injured from Iraq. I know many of you have vast experiences on TBI. I need some help and inputs for those with vast TBI experience or anybody who can just help. Poor guy has limited time in our program and I want to aggressively throw out everything to help him. Pt has poor cognition, inconsistent in following 1-2 step commands, has agitation spells but could be redirected, double amputee BKA-AKA, extensor tone interfering in sitting balance, catheter and colostomy bag. I was thinking of having him on a temporary prosthesis, strap him to a tilt table and start standing. Maybe having him prone would breaks some tone on his extensors and improve sitting balance? Or have him in a vestibular ball will help? Is it justified to wait for the cognition to improve then proceed with all of these aggressive methods coz we are basically doing nothing now but ranging. I need to present to our team first before I actually do this. Please share your precious knowledge to help this guy. He just received a purple heart medal.
