Answer:  If you noted the space between the 1st and 2nd metatarsals were abnormally wide...you were correct.  This is the classic...and very painful Lis Franc fracture-dislocation.
It is often missed...very painful and a large amount of swelling.
Lis Franc’s fracture-dislocations can occur in many different ways. It can be caused by both a direct crushing type injury or a force applied to the metatarsal heads (ball of the foot) which both can result in displacement of the Lis Franc’s joint or fractures that in involve the joint. Common causes are motor vehicle accidents, falls from heights, severe foot and ankle sprains, crushing force to the top of the foot

Also bonus points to those that noted this was of two different feet, one left and one right (couldn't find the correct xray).

This type of fracture, if missed and noted followed up on, could cause a lawsuit...and great disability for the patient.

This is a real case.
