However, once the cortisone shot wears off, they will eventually come back, especially if they were continued to aggravate the elbow.

 lateral epicondylitis, when more chronic can be quite debilitatiing.  My suggestion, as the typical palliative and modalities ( rest, US massage etc) have not been effective is to 1.   recognize this problem may be more degenerative i.e tendonosis, rather than a more acutely injured area.  Also  remember that there are >20 causes of lateral elbow pain.  I suggest, if you haven't already, getting examined by a  expert in the field.   ruling out cervical problems, radio-capetellar OA, and other  conditions can help you focus on the issue. 
If it truely is recalcitrant lateral epicondylitis,  consider  eccentric exercises, a continuation of stretches and a  tennis elbow brace if helpful.

I am suffering as well...I have had soft tissue work (too painful and counterproductive), ultrasound, ionto, mobilization of the radial head (painful)...the biggest help was laser therapy...but I still have the pain 2-3 months later.  Time for the shot.
