I think this person was ambushed by an IUD-weilding USPS man enraged with his profession. Who knows for sure though. ;)

UTDC -> Re: Chronic lower back pain (August 11, 2005 7:23:00 PM)

LOL. Sounds like a classic episode of Seinfeld with Newman on the rampage.

JLS_PT_OCS -> Re: Chronic lower back pain (August 12, 2005 2:17:00 AM)

Jeff-
If you've got a test for IUD instability, I think you might best keep that to yourself, buddy...

Moderator: I apologize for the downward turn of this thread.

Jason: That was freakin hilarious.

Alex doesn't mind a little rambling here and there...do ya Alex? :)

Alex Brenner PT MPT OCS -> Re: Chronic lower back pain (August 12, 2005 12:24:00 PM)

Of course not.

jma -> Re: Chronic lower back pain (August 12, 2005 4:13:00 PM)

Hmm, so what are the findings on this film, Alex?

Synergy -> Re: Chronic lower back pain (August 12, 2005 4:54:00 PM)
Do the cuboid whip and all is well.
ROFL! I guess I missed that earlier Greg.

Jeff,

You mentioned a 'probable bone island in the body of T9'. Without researching this, what exactly is it? I believe I may have seen one of these today at work. One of the chiros had me come in the x-ray room to check out some poor woman's films. She had the exact same thing, but it was quite a bit bigger.

Thanks!

UTDC -> Re: Chronic lower back pain (August 12, 2005 5:54:00 PM)

Hey Chris,

A bone island is a benign entity comprised of a focal area of cortical bone within an area of medullary bone. Since cortical bone is more dense, it appears more white or "radiopaque" on plain film xray. It's really nothing to worry about, oftentimes radiologist don't even comment on it. The tricky part is differentiating a bone island from other, more concerning entities such as blastic metastasis- hence the "probable" modifier on my comment. Although there is such a thing as a "giant" bone island, I would have your chiros films read by a radiologist before assuming it is a benign process.

Jeff

PS- what was the chiro's assessment of the xray?

jma -> Re: Chronic lower back pain (August 12, 2005 5:56:00 PM)

I agree.

Synergy -> Re: Chronic lower back pain (August 12, 2005 6:19:00 PM)

Thanks for the info Jeff! :)

There is a spondylolysis, bilateral pars interarticularis defects, of the L5 vertebrae with a very minimal spondylolisthesis.

There is a small troll pick axe in the lower abdominal region. Just joking, it is an interuterine device.

The L5 SP, pelvis, and SI joints are WNL.

Good job Greg.

chiroortho -> Re: Chronic lower back pain (August 13, 2005 4:49:00 AM)
small troll pick axe in the lower abdominal region
Funny.

Could also be the handlebars of a pixie's razor scooter.

jma -> Re: Chronic lower back pain (August 13, 2005 7:23:00 AM)

Thanks for the update Alex. Nice highlights. Keep them coming.

ehanso -> Re: Chronic lower back pain (August 13, 2005 2:23:00 PM)

So is she missing her "scotty dogs"?

chiroortho -> Re: Chronic lower back pain (August 15, 2005 3:18:00 AM)

Falls, the scotty dog refers to the appearance of the area on an oblique view. If there was an oblique view here you'd see what is meant.

It almost looks like the bottom pic represents a badly positioned oblique but I doubt that this was the intention. The reason that I say this is that you can see some rotation of the vertebral bodies, kind of halfway out of lateral into oblique but not quite. In a big person positioning can be tough (not calling this lady a big person of course).

chiroortho -> Re: Chronic lower back pain (August 15, 2005 3:21:00 AM)

Here's a scotty dog looking to the left for you. Don't know how to doctor it up with arrows like Alex, but I think you'll see it when you look.

ehanso -> Re: Chronic lower back pain (August 15, 2005 3:43:00 AM)

Thanks for the view Greg. I realize that it is only seen on an oblique. I was trying to add a little tongue in cheek humor. I have had some success treating these with core stabilization. What do the rest of you guys/gals use?
Alex, Keep them coming.

chiroortho -> Re: Chronic lower back pain (August 15, 2005 5:30:00 AM)

Sorry Falls, didn't know you were kidding...just trying to be of help.

Greg
