Jeff,

You mentioned a 'probable bone island in the body of T9'. Without researching this, what exactly is it? I believe I may have seen one of these today at work. One of the chiros had me come in the x-ray room to check out some poor woman's films. She had the exact same thing, but it was quite a bit bigger.

Hey Chris,

A bone island is a benign entity comprised of a focal area of cortical bone within an area of medullary bone. Since cortical bone is more dense, it appears more white or "radiopaque" on plain film xray. It's really nothing to worry about, oftentimes radiologist don't even comment on it. The tricky part is differentiating a bone island from other, more concerning entities such as blastic metastasis- hence the "probable" modifier on my comment. Although there is such a thing as a "giant" bone island, I would have your chiros films read by a radiologist before assuming it is a benign process.

There is a spondylolysis, bilateral pars interarticularis defects, of the L5 vertebrae with a very minimal spondylolisthesis.

There is a small troll pick axe in the lower abdominal region. Just joking, it is an interuterine device.

The L5 SP, pelvis, and SI joints are WNL.

Falls, the scotty dog refers to the appearance of the area on an oblique view. If there was an oblique view here you'd see what is meant.

It almost looks like the bottom pic represents a badly positioned oblique but I doubt that this was the intention. The reason that I say this is that you can see some rotation of the vertebral bodies, kind of halfway out of lateral into oblique but not quite. In a big person positioning can be tough (not calling this lady a big person of course).

Thanks for the view Greg. I realize that it is only seen on an oblique. I was trying to add a little tongue in cheek humor. I have had some success treating these with core stabilization. What do the rest of you guys/gals use?
