Steve, The normal film (which is the last film that was posted) is with the arm down by the side so no they are not exactly the same view as the original case study film posted above it. I will try to post a normal with similar degree of abduction to compare with.

While these are interesting (at least to me), I don't think they exactly answered your question Alex. I've spoken with one of our orthopedic surgeons who views the data linking acromion type to pathology as "soft".

This is a better normal radiograph that is a more similar position as the case study to compare the downward sloping of the acromions.

Is a downward sloping acromion the same as a type III acromion (Bigliani classification)?

Alex, I'd guess that a downward sloping acromion is not the same as a type III acromion. I thought the Bigliani classification dealt with the degree of "hooking" of the acromion, not the slope of the acromion. Biomechanically, would the slope be more indicative of the scapula position?

Interesting. I thought that as well with the hooking of the acromion.

Type III is also described as 'hooking'. Could be understandably confusing though in that some might confuse a subacromial spur as hooking, and a spur as you know is a different animal.
