These postings labeling those of us who spent time in long term care as "strippers" make me very uncomfortable and angry. I spent over 30 years in long term care but my reason for being there was not the money it paid, it was the draw I had towards being with and working with the elderly. Not many therapists had the interest or patience in this area....it wasn't an easy area to work in unless you had a real affinity for the elderly. Yes, the pay was good, it was very good...but to call me a "stripper" for providing the care I did is a professional and personal affront and I am deeply offended. I never gait trained those who would not walk again, I did not exercise those near death...... I transferred exercises and mobility in those cases to nursing programs. I understand that many of my peers did...but I refuse to believe that the majority did.
I had worked for large corporations all of my professional life...only one wanted me to "recycle" patients through Part B and keep patients on Part A beyond what they qualified for....and in this one instance my resignation was requested when I refused to do this...and it was received. This was once in 30+ years..not too bad a record for the accusations being made.
I know there are "horror stories" of Medicare abuse by contract companies (worked for one and found them very much in compliance) and therapists...but the abuse is found everywhere in healthcare and we should point fingers there also and not just at our peers holding them almost solely responsible for our situation. Medicare has been abused since its inception around 1965 by many disciplines and companies.
I was a new therapist in 1965 making a whopping $7600 a year... BIG MONEY at the time . I was offered a job at a new nursing home paying $16,000. When I asked how they could afford to pay a therapist TWICE a high norm the reply was " There's no problem, Medicare will pay for it.....". I turned the job down saying to myself "Medicare will never last....". I was intuitive about Medicare...only 30 years too soon.

Rose (etal),
I certainly was NOT comparing you or anyone else in this forum to a stripper. I was trying to make a point that if morality or ethics did not play a role in patient care, then I might as well be a stripper. I would think (and hope!!) the money would be better. To only be a PT for the money is WRONG!! To be treating 6 patients/hour without doing progress notes, re-evals, or discharge summaries is not what our Code of Ethics is about (nor do I believe it is legal). To boot, patients in JoePT's clinic are seen for 2-3 years!!
Sure, I would love to make more money. I am no different (I'm sure) than most people who have various financial woes. But to sell myself, my patients, my peers, and my profession for a dollar is not right. I have had the opportunity, just like you, but chose not to take it.
I do apologize if anyone got the wrong impression re: my previous posting. I just hope that JoePT and JadedPT reflect on the decision they make everyday when they go to the factory they call work.

Rookie, You asked what type of facility I work in. I work at an privately owned (by a businessman) outpatient ortho clinic in a big city. Interesting story today. Due to the meeting you had with your boss, I was inspired to do the same with mine. I spent my entire time this past weekend developing a plan in which I could explain to my boss how we could save more money in the long run if we improved our paperwork in order to ensure payment and avoid auditing, etc. I was hoping to schedule a meeting with my boss today for sometime during the week. Unfortunately I made the mistake of telling JadedPT when I walked in this morning. He got a big kick out of it and was laughing about my plan and this website with my boss and another PT. When I walked in the lunchroom I could tell they were talking about me. Then JadedPT says, "Hey boss I know how we could save the clinic some money. We could fire Joe and hire a newer grad for $10,000 less." Then my boss shot back, "No need for that. I am a fair boss, I don't fire my employees. We'll just have Joe see 8 patients an hour." Then they all started laughing at me. Later on they all said they were only kidding, but I was still humiliated. You all have good points on this forum but they just dont seem to really work in the real world.

Gosh, it seems like some of you feel trapped in a cage of your own design.

Joe,
You may think that you're in "the real world" but that would have to make you wonder where the rest of us are living.
Personally, I think you're living in the middle of a novel by Kafka. This would account for the demented values of your employer and the sense of, uh, humor displayed by your co-workers. Assuming you've described the situation accurately, they aren't actually therapists in the sense that they are curious, committed or caring.
Maybe you should just try one simple question of them: Would *you* want to be a patient in this clinic?

Hey Rookie, I've got a great idea for us. Rather than get depressed over the state of affairs in our clinics and the impossibility of exacting a change, I have a great business proposition for us to become independent contractors. We can contact various strip clubs, Hooter's Restaurants, modeling agencies, etc. We can offer our therapy services to any employees that might get injured on the job. Keep in mind, it can get dangerous swinging on those poles or walking down a runway. This way we would never complain about seeing more than 6 patients per hour Let me know what you think.

What happened to the Bird, did i ruffle her feathers??? Maybe she is busy seeing all those patients?? Hey joe, sounds like a great idea!!! My wheels are turning, we could do some ergonomics and make sure all the poles are the appropriate height as well as looking at gait analysis while on the runway. This sound like we would be providing a much needed service to a scorned profession.

Barrett hit this one on the head.
I would add only this. Too many use "I'm just living in the real world" to excuse behaviors they can not ethically justify. Gang members who shoot each other are only living in the world that is real to them. Those addicted to drugs who steal and kill to support their habit are only living in the world real to them. Politicians who point their finger at you and lie without blinking are living in the world real to them.
Be careful the company you keep.
