Sarah, if he has had pain starting 4 months ago, the original, primary "injury" ( asimple possible mild capsulitis from overtraining or straining) may have resulted in a non-resolving adaptive muscle pattern. Which in itself is now the culprit of maintaining discomfort in the area by the changed arthrokinematics. Not often, but i have occasionally seen these. Have you applied axial pressure through the femur with the hip in 90 flexion and 30 adduction - moving it through to 30 degrees flexion (while compressing lightly? This "clearing" test usually does NOT aggravate SI joints, but will aggravate hip joint dysfunction - indicating an irritable joint. And finally, it CAN be OA. The youngest one I have seen was 19 years old. Mind you, this is in 23 years of practice and the ONLY one that young - so take that with a large salt shaker!

My lumbar spine is not that bad. Whenever I get achy my left side is the usual culprit. ROM is full-painfree usually, It is interesting you ask though, b/c recently I was showing a patient a exercise in s/l and reached back to grab the tx table and pop, click, click and pop all coming from my back. Sort of tough to evaluate myself but since I have been achy lately I am going to have someone at work take a look at me. The last MRI I had showed some mild DJD L4-5, nothing impressive. I have also noticed that when I treat lately, I am putting my left foot on a stool to take the weight off, which feels better.

Sarah-I just had a 13 y/0 old with HNP (trauma) and one of my colleagues had a 16 y/0 with "DJD" lumbar spine, according to the physician. Not sure how that panned out since she was not my patient.

The patient in question is progressing very nicely, so I am inclined to go with Sebastian's theory of the non-resolving adaptive muscle pattern. And yes, the scouring test was negative for hip joint dysfunction.
