When do you consider electrical stimulation or PRE exercises for neurologically weakened muscles secondary to a disc herniation.

Do you start this immediatley or wait to see how lumbar treatment resolves myotomal loss.  FES versus PRE's - better results with one or the other

I do both....not overloading at first (usually just isometrics) and then progressing with PRE's, and using ES when tolerated.  I find this works much better, with less residual weakness, than starting on those who have atrophied and weakened over months before seeing me

How long do you give them to regain strength with focus on treating LS before starting isos or e-stim.

never.

the disc is not the source of the problem. the symptoms are from a spasm pattern that is so easily missed and most often not treated correctly.

come out and get a lesson or two, I'll have you going down an entirely new and better path. sure to blow your mind. I share my office w/ a pt, she'll tell you.

Kaden:  It depends on their symptoms.  Those with signs of severe HNP or neural encroachment, say in the neck, to the point where they come in with their chin on their chest and inability to lie prone won't get started on these until they can at least get some pain relief.  Those with more minor symptoms, often not even realizing they had myotomal weakness, get started right out of the gate.
