Does anyone have a good reference for wording of a consent to treatment form? authorization and assignment form?
I am working for a new company and, although I have found a consent to treatment form is not legally necessary, I have always used one in the past and would feel more comfortable (I just didn't snag a copy of one from my last job). I would also like to reword our authorization and assignment form. Any ideas of where to go?
