I don't have a specific protocol. In general, prior to 6 weeks I focus on posture, education about positioning, soft tissue work, scapular strengthening and modalities as needed. Once I get MD clearance, usually 6 weeks or so, I start some very gentle AROM for the cervical spine in all planes but especially rotation. The fusion should be "taking" well enough to not only start AROM but some gentle cervical spine stability work. After 8 weeks or so, if they have some stiffness remaining I will start gentle stretches and progress cervical and scapular stability work as tolerated. Don't forget to look at joint mobility especially in the TS and upper cervical spine. Sorry if this is more simple than you were expecting. Overall, I find these folks do really well as long as you respect healing times, progress them gently, and educate to decrease their fears about movement and overall recovery.

Thanks Kaden. He had surgery 3 months ago. It wasn't until he went to his doctor 2 weeks ago that physiotherapy was recommended. I saw himfor the initial assessment last week. It's been awhile since I've had an anterior cervical fusion. Do you manipulate the thoracic spine?

I commonly see referal to PT this far out. Nice in a way as there should be excellent healing and you should be able to make nice progress. I would manipulate the T-spine with arms crossed over chest and head supported on a pillow. I tend to avoid the behind the neck position.
