I was wondering what the consensus is on wearing a brace after spinal fusions, especially mulitple levels?  I work with one neurosurg who is impartial and one who gives it to EVERY patient.  It seems that for years patients were not receiving the braces and were doing just fine, why the new transition?

There appears to be no evidence for it based on the one summary I could find (2005).  Still would like to know others' opinions.

Although conflicting reports have been presented in the literature regarding the utility of lumbar braces for the prevention of low-back pain, most Class III medical evidence suggests that these supports used prophylactically do not reduce the incidence of low-back pain or decrease the amount of time lost from work in the general working population. Among workers with a history of a back injury, their use appears to decrease the number of work days lost due to back pain. Lumbar braces appear to be an effective treatment for acute low-back pain in some populations. They do not appear to be effective in the chronic low-back pain population. If a brace is used, rigid braces offer some benefit over soft braces. There are no data to suggest that relief of low-back pain with preoperative external bracing predicts a favorable outcome following lumbar spinal fusion. No information is available on the benefit of bracing for improving fusion rates or clinical outcomes following instrumented lumbar fusion for degenerative disease.

Most neurosuergons I work with brace in some manner.  However, I don't think bracing for the first 6-8 weeks necessarily prevents movement at the fusion site, improving healing. 

I view it more as a proprioceptive reminder to patient to not "overdo" range of motion.  We know from studies that ankle taping does not necessarily provide enough protection to stop the ankle from "rolling" but from experience I see a lot fewer injuries from athletes that tape on a regular basis.

i was told by a really reknown surgeon at hopkins "i did all my stabilization inside" why would a patient need a brace...

i agree, bracing maybe a good reminder for a patient, but i mean is a patient really going to disrupt a fusion, ihave never heard of it happening

I have never heard of a fusion being disrupted either.  I had one patient slip and fall a few weeks after a cervical fusion (not braced) and everything was OK.  It seems it would be very difficult to harm.  When a patient is wearing a brace for 95% of the day for 6-8 weeks, there are many additonal issues that now need to be addressed.
