Hi all,
My CI and I recently examined a 23 yr. old athlete with a pronounced bunion on the R foot that is causing pain under the 2nd methead after activity. Pt. reports that she does not frequently wear shoes with a tight shoe box. Inspection of the shoes she wore to the clinic revealed significant wear of the lateral heel. Other clinical findings include: mild forefoot varus on the R, a dorsiflexed 1st ray on the R, crepitus with plantar flexion of the 1st ray, calluses under the 1st and 2nd metheads on the R, decreased dorsiflexion on the left in subtalar neutral, and bilateral hypermobile feet joints. This patient would like to return to running 20 miles a week without pain or further damage to the 1st ray. We issued her semirigid orthotic inserts to control pronation, but other than that, do you have any tips that I could pass along to her?
