Outside of arthroscopic surgery, what are the treatment options?   Does it help to know if it is due to impingement?

I've read quite a few articles on the problem, and PT is always attempted unless a very severe tear.  Surgical outcomes may not be as great as first touted, but sometimes there is no choice.  Yes, FAI is often a cause of the problem, but even a recent article about this condition noted that PT can just agitate.  So, if treating, cause no pain, use common sense orthopaedic principles and at least not make them worse

Actually, I covered a Marc Philippon article from the journal Pittsburgh Orthopaedics on hip labral injuries.....it was so long and detailed, I had to break it up into 3 parts.  While he has since moved from this area, I have had some patients travel to have the surgery done.  Interestingly enough, I've also had more than one other ortho indicate that while he was doing surgery on just about everybody, the results were not all that spectacular.  It does not seem to be as easy and successful as trimming a frayed meniscus.

I'll look into finding some articles by Dr. Philippon.

As for PT, would I be right in assuming that therapy would be based around mobility of the hip and reducing femoral anterior glide (hip flexor stretching, gluteal activation, etc)?  What would be some common recommendations?


I apologize if any of what I am saying doesn't make sense, I am self taught as far as this stuff goes.  The best I have to go on as far as biomechanics goes is the research that I have done as a competitive strongman.

Take some movement baselines and look at repeated hip extension in half kneeling then reassess baselines.

This is what I was looking for.  I'm not looking for an over the internet exercise prescription, rather like a "most common treatments" or "what to expect".  I won't be able to see a therapist for a few months till I return from my internship, so I'm trying to get an idea for what is out there or what the general procedure is. 

If I was to stop lower body training, provided it isn't completely destroyed, what chance would I have at recovery?
