I've read some forums recently where OTAs have been freaking out about some sudden crisis in their profession and a lack of jobs.  Has a change in the health care system caused OTAs to be edged out by OTs?

If so, do you foresee the same happening with PTs and PTAs?

I think there will always be a place for PTA's.  It's hard to find another associate's level program where you can make that kind of money.  However, the money PTA's are asking for are approaching the level of a PT.  All things being equal, or even if the salaries are close, I'd have to consider the PT from a business standpoint.

Yep, teach, they very well could be in the outpatient setting.  Our economy sucks... health insurance coverage has higher and higher deductibles and co-pays... this leads to a potential change in utilization of physical therapy services.  The utilization change will inherently be less frequency of visits, possibly longer appointment times and a longer duration of an episode of care.  With that change, the therapist's role will change and evolve to be more "consultant" like or more of a high reassessment and continue to manage the patient's progress and updating the program for patients to meet anticipated goals.  With a higher amount of time spent reassessing, there will be less need for a PTA because a PTA isn't going to have the training/insight to do what needs to be done adequately.

SJ,

You have talked on a couple of threads about the "consultant" role.  I for one think it would be great if PT went this direction.  One, it would allow for the more evidenced base efficient PT to thrive and would be a win for the patients.

Two, I think we could then finally be looked at as more autonomous practitioners and the expert of choice in MS care.  We would be looked at more as the blue collar expert consultant as opposed to the skilled laborer that we are often perceived as.

I have had patient for one reason or another that could only come in once every two weeks and so that is what was done.  It has really made me look at some of my treatment ideas when a typical diagnosis I would see 2x a week for 4 weeks I instead saw 4 times over an eight week period with similar outcomes.

... and yes I do think PT will eventually go this route and the PTA could find themselves on the outside looking in.

In our area, there is actually a high demand for COTA's.  However, there is not a nearby school where people can go through a program.  We used to have one at Penn State, but it was closed down years ago.  There is also a high demand for PTA's being that the Practice Act is changing here in Pennsylvania.

But who knows where this economy will go and what will happen.  Until then, I'm not worried. =)
