Hello,
A colleague of mine called to ask what an ankle decompression is. From what I was told, the pt sustained an injury, that he was not aware of, in the ankle that happened 10 years ago. An incision was noticed in the lateral side of the anterior surface of the tibia. The surgical site consisted of a longitudinal scar about 1/2 posterior to the medial side of the malleolus. The pt c/o numbness in the bottom of the heel since the surgery and ambulates with toe touch WB because pt cannot sense heel strike when walking. Pt has a stiff ankle with all active movement and is limited in DF. Pt had the surgery in the beginning of 2003 and the numbness has not changed. Dorsal pedis pressure was 2+ but achilles tendon reflex could not be done secondary to DF restriction.
Has anyone treated a pt with an ankle decompression. Unfortunately, this pt arrived late, so the surgeon could not be reached for further clarification. Ideas on what was done surgery wise? Pt, before coming to therapy, explained the numbness to the surgeon and was told to go to PT.

Hello again,
Perhaps I wasn't as clear. So I will rephrase the question.
Has anyone seen a pt with an ankle decompression? If you have, please tell me how they presented and what was done PT wise. Your response will be greatly appreciated.

Decompression is intended to relieve pressure on something. Usually, it implies nerve when performed on the spine, but can also be other soft tissues, such as a subacromial decompression for impingement. If there is an incision posterior to the medial malleolus, a potential reason for this surgery was to decompress the posterior tibial nerve. However, they may have been cleaning up scar tissue, which might also help to relieve pressure. All of this is speculation, as I dont know what symptoms were prior to surgery. It is imperative that you know as much as you can about what was done before you proceed, as is the case with any post-surgical rehab.

Perhaps I am just reading it wrong. How many incisions are you noting? And where exactly is the surgical site. This would help. As the previous poster said....if it was done posterior to the med mal....then it could be posterior tibial nerve decompression. However, it sounds more like they affected it during the surgery.

I would talk to the doctor and/or see if you can get a copy of the operative report. I have not heard of ankle decompression that much. YOu can get impingement from bone spurs on the malleoli.....

Hello Mcap,
There are two incision sites. The main incision site is the scar behind the medial malleoli. The other one was medial to the anterior tibia. The pt told my co-worker that something was taken from there and placed in the ankle and a bone in the ankle was taken out.

My co-worker could not reach the surgeon but a request was faxed but nothing was sentback. Hopefully, the surgeon could be reached to get a operative report.
