Are there any studies to support palpation as a tool to assess vertebral instability?
Validity? Inter-rater reliability?

Thats a good question. I would guess there is not much support. However, you must define instability first.

I just viewed the MRI tonight with a neurosurgeon for a friend of his. This 60-something gentleman has horrible degenerative changes. In one lateral view, you can see the bony changes that the surgeon told me indicates instability-an odd discoloration of the vertebral bodies spreading out from the endplates. However, mobility assessment at that level to me indicates hypomobility. So, the question does beg to be asked "how good are we at really detecting this?

If we assess and feel it is hypomobile, and mobilize when actually hypermobile and unstable, are we doing more harm than good? How many PT's are mobilizing a tight shoulder that really does not have GH capsular hypomobility, but rather AC joint problems, or some other neuromuscular situation that has tightened the shoulder? Are they doing something that will create problems down the road, such as secondary impingement?
