I tore my left ACL ligament four days ago playing in our post championship softball game. I am really upset about it. Sucks.

I lead off with a single in the top of the second inning, moved to second on a tag up on a deep fly to right. The next batter hit a fly to left center. I was on second and was going to fake tagging up to force a throw from the outfield to third. When the outfielder caught the ball I came off the bag hard and took three steps towards third and then planted my left leg to stop, pivot and come back to the bag. My knee gave out, I heard and felt a loud snap. I fell to the ground about three steps off second base. I tried to get up to get back to the bag and the knee gave out again. I was tagged out, we lost in the end. Sucks.

I thought I was ok because there was not immediate swelling. I play on my health clinic team along with some fellow docs so I had one of them perform a Lachmans. He initially thought he felt an end feel but as I was watching I noticed too much translation as compared to my right. I later had our ortho PA check it and I was able to tolerate a pivot shift.

It surprisingly was not too painful. I had about a 6/10 immediate pain which resolved to a 3/10 in about 20 minutes. I was able to ambulate independently on the sidelines 20 minutes after the tear. I had slow onset of about about grade II effusion and I walked on it that night without too much difficulty. I iced and elevated all night, quad sets and SLRs with minimal pain or difficulty. I went to bed and the next morning I had grade III effusion, wicked quad inhibition, and I could hardly walk. My motion was about 10-40.

I currently have no pain except pushing into flexion deficit (about 3/10), AROM 5-0-100, good quad tone, SLR no lag, Grade II effusion. I have positive Lachmans no end feel, positive pivot shift. No joint line pain, Negative Thessaly, no deficits with V/V.

Did I mention that this sucks?

Alex,
Are you sure you didnt do it that time you saved that busload of kids from plummeting off the cliff? Its so much more glamorous.... do you know any good PTs? and when is your MRI, then surgery? I hear that injury sucks.

Alex, That really does suck. I hope things go well for you.
I had a pt one time who I was seeing post ACL repair. He told me he did it in a football game. I took him at his word. 3 weeks later his wife is with him and asks if he told me how he hurt it. He looked really sheepish as she said he was coaching a powder puff tag football game and one of the women fell into him on the sidelines. LOL

Jason,
Great question. I have been asking all of my PT friends in Europe and most seem to prefer the HS graft.

I personally think the BTB is the way to go since I play a lot of sports. I am a fairly large guy, 6 feet tall, 198 pounds so I will need something strong.

What do others think? BTB or HS graft?

By the way, the "pop" that most people describe was actually a snap in mine. If you you slap your hand against the beefy part of your forearm that is what it sounded like.

Alex,

I did my right ACL in about 4 years ago - audible pop while walking in the clinic, absolutely no end feel with lachmans (The fellow very experienced therapist who did the test had his own apprehension reaction) and confirmed by MRI. After weighing the options and rehabing myself I opted not to have it repaired. 4 years later - no problems, able to jog 10 km but no return to rugby or soccer.

ACL tears are actually able to be spontaneously healed by a few minutes of mobilisation to the ipsilateral facet joint at L3-4.

I'll be interested to hear how it goes for you Army, I'm a big guy too (6'5", 220) and know somehow I'll blow an ACL somewhere down the road.

Alex,

Are you going to get some con leave out of this? Seriously I am sorry to hear about your knee. I would come back to the states to have this done, any connections at West Point? With your size and desire to continue sports I'd vote for the BTB.

Sorry to hear that Alex and yes...it does suck! I say go for the allograft. Of the ACLs I've rehabbed, I feel the allografts have done better in terms of a more expedient recovery and faster return to activities, but I doubt I've seen as many as most of you here. :)

Alex , Do you remember Dr. Bottoni from West Point? He is now at Tripler in Hawaii, I'm sure he'll do the surgery. Hawaii is nice this time of year :) .

Sean, LOL, maybe the 'pop' was a referred sound from the facet joint. And he is now experiencing psychosomatic swelling and instability mirroring the true injury at the facet joint.

so what happens now to your position? do they fill it, or do they send troops else where? do they have you rehab it there or somewhere else?

I have seriously altered the things I do, because I cannot afford to be on crutches, etc. Not rubbing it in, just I could not even imagine a serious long term recoop type injury right now, I have stopped skiing and stopped jumping into volcanoes to keep a bit safer-
