When treating "Tendonopathies" do you attempt to differentiate between tendonitis and tendinosis?? There has been several journal articles about this as of late. What do you think and how do you treat them differently.
I definitely have my own thoughts/techniques in this area but I want to see what you guys/gals think first.

Tendinosis: intratendinous degeneration due to atrophy asymptomatic, nodule, or point tender. Tendinitis tendon strain or tear variable degree of damage to tendon, inflammatory response. Program. eccentric loading dat 1-2 slow contolled day 3-5 rapid, controlled day 5-7 drop and stop or drop and reverse. Progress by speed and resistance. I agree with the eccentric program.
The bottom line is that they are far different entities. I treat Tendonitis with rest, modalities, medications (per MD) and maybe stretching.
Tendonosis is actually much more common and I treat it agressively with friction massage and repeated loading (eccentric as the progresses). The idea with Tendinosis is to stress the tissues and alter the way the collagen models as it heals.
Is this scientifically based, only partially. There is great scientific evidence for differentiation of the two types of tendinopathies. But there is only a little research in terms of outcomes when they are treated separately.
Mark Laslett proposes a good system for eval and tx in his book Mech Dx and treatment of the upper extremity.
