The procedures I have watched flip the patella laterally, therefore stretching/detaching the medial retinaculum and surrounding tissues. Perhaps that is one reason. Another could be a soft tissue strain due to mechanical re-alignment, as they remove either a severe varus or valgus deformity. It could also be cutaneous nerve stretching from the procedure.

I second Mr Duffy's response. There is a branch of the saphenous nerve there, and these surgeries are never a gentle procedure. The pes anserine is there, but I think actual bursitis of the pes is fairly infrequent, rather it is referred pain from all of the other possibilities that John noted.

Hey, just because I turned 40 on Tuesday does not mean I deserve to be called MISTER Duffy!!!!! After all, I look 25 and still like to shoot spit balls.

Here is an article that might be of interest:

SAPHENOUS NEURITIS: A POORLY UNDERSTOOD CAUSE OF MEDIAL KNEE PAIN. Journal of the American Academy of Orthopaedic Surgeons, Vol. 10, No. 2, March/April 2002.

Just respecting my elders John. It refreshing to see that Im not the only one who acts like a child. I had a dart gun fight with my 5 and 10 year old last weekend.

Thanks for the reference by the way.
