I had a client go to a physiatrist for emg/nerve conduction studies for his lower extremities. Mechanism of injury was 10 foot fall off of a ladder onto his back/buttock. Had significant bruising. The emg/ncs came back normal but the physiatrist diagnosed him with a S2,S3,S4,S5 neuropraxia related to a retroperitoneal hematoma. The MRI came back negative for a hematoma soon after, and is showing some degeneration and a hamstring tear at the ischial tuberosity. He's now 4 months post and complaining of isolated numbness at the posterior thigh. He walks with an antalgic gait, with the right foot significantly externally rotated and complains of his right leg giving way. So my questions are: 1) is the retroperitoneal hematoma and sacral neuropraxia plausible? 2) What do you think is accounting for the gait changes?
