One quick note! From what I found out, there may be a new 3rd edition coming out late July 2006, I think. You can find that info from this website: http://www.us.elsevierhealth.com/product.jsp?isbn=0323032842

If you go to the APTA (American Physical Therapy Association) website. On the left hand side of the home page, you will see a link called education. From there you can look at schools in the NY area or anywhere else. Each school should have a link where you can contact a PT program, either by phone (preferred) or by email, and ask for a information packet. Each PT program has its own applications process and reqirements for their programs. Get a couple of them and you can compare and see the similarities and differences in the application process. Not all are the same and some ask for more and some ask for less. See where it takes you.
Go to this link, from the APTA website, to get started:
http://www.apta.org/AM/Template.cfm?section=PT_Programs&template=/aptaapps/accreditedschools/acc_schools_map.cfm&process=3&type=PT
