I just started an outpatient physical therapy clinic in upstate NY in December. I recently had a Personal Injury attorney who specializes in MVAs refer me a client for a functional evaluation and report. Not having dealt with the intake of these types of cases… I have quite a few questions that I would appreciate feedback on.

I don’t think the patient has a MD script for a PT evaluation (I’m awaiting the client’s full medical reports). Am I required to have a MD script for a PT evaluation since NY is now direct access?

The attorney’s office asked me my rates for an evaluation and report. Does anyone have advice on what the going rates for an evaluation and report for PI cases are?

The attorney’s office said they would pay me for the eval and report. Should I have a written agreement with them?

Does anyone have a template for a PI report that they would be willing to share or direct me to where I may be able to find one?

Are you trained in FCEs?

If this was me I would be sure to have a physician referral to cover all bases. I have performed hundreds of FCEs and make it a point to have a physician order before proceding.

There are no specific reports for PI. If they are asking for an FCE then there are reports available. I would make sure that you are doing a defensible FCE and not a home grown type that so many clinics complete.
