My hip has been killing me and I'm trying to figure out what could be my problem. Pain with ER at end range. Pain in the groin so I'm sure it's something in the joint, but not sure what it could be.

No antalgic gait, no limp. Just the pain--relieved with position change.

Have had the pain for a while. ROM appears to be WNL.

Im not sure I understand. Is it ONLY killing you at end range ER?

I can feel pain increase as I ER the hip in any position.

Ok, not really killing me, but say 6/10 at end range of AROM, PROM is painless--must be muscular. Oh yeah, ER is 20*, IR is 3*--I think I need to work on that, lol.

Sounds like a capsular pattern. How does your other side hip ROM compare?

need plain films to r/o OA, AVN, Femoroacetabular impingement, femoral neck stress fracture ( if recent increase in milage) iF you are young and active and had a recent history of "groin injury" that did not improve MR Arthrogram is a gold standard for labral conditions. Chondral and labral injuries are becoming more common when there are refractory hip cases.
Consider SI mediated pain if only at end range also.
