I'd like to know where I can find the eval and weekly forms for outpatient clinic?
I'm trying to start a small clinic and I don't have the forms. Anybody?

Where are you opening your small clinic? What format do you require, excel, word, infopath, or another software. Do you plan to hand write your evals or electronically?

I'd like to start in the northern subarbs of IL. Word format would do.
Also, could you tell me how you do outpatient rehab at home?
Are you affiliated with in-home rehab?

Sorry for the late response. I have been real busy and haven't had the chance to reply. I am not affiliated with IN home or Fox rehab. I'll send you the forms at your email. In fact you can even download one now at the microsoft office template download page. I just want to clarify that I perform my treatments at the patient's home, not mine. Though I could but that would require some remodelling and zoning investigations to say the least. Having said that I have enrolled with as many local health insurance companies as I can. I have a clinic so my at home therapy goes under that tax ID. getting a tax id is a good choice. I don't know about IL but in NJ single member LLC taxes are just like sole proprietorship, meaning pass through taxes (schedule c), which is great.
If you have time to keep want to keep good accounting reports thats up to you. If you want to do cash based thats great. I have purchased equipment that is portable and what I like to use with my patient population. If you are interested I can post a list. I don't want to bore everyone with an equipment list. I make my own marketing materials thanks to todays wonderful printers. And I get patients from a lot of sources. From phone calls, word of mouth, marketing to internal med doctors/ family/ endocron/ ortho/ geri/ gastro/ cardio/ DPM/ and nurse practitioners. I also have a clinic so I market my outpatient clinic and I say that for the patients that are not homebound, home care benefit can't be used but please send them to me and I can arrange paratransit for them or will go to their home. I don't want to get off the topic here so If you ahve any specifics you want to discuss bring them up.
