Saw a patient today, young girl (16). Saw her for thoracolumbar pain. She has tight muscle belly in r side paraspinals. I was working on her exercises when I realized her s/c joints are subluxing anteriorly with any ue movement over approx 120 degrees. No pain with sublux, and she says it has been happening for as long as she can remember. Mother never noticed it and referring MD didn't notice it.

I notified the MD and he just told me to "fix it" and thanked me for calling him.

Now, how do I fix it? I tried MWM and during the movement there is no sublux, but there is pain and sublux returns after removal of force.

Anyone ever experience this?

This is a just a guess, because i haven't heard of this before, BUt i could have to do with the articulating disc between the clavicle and the sternum. Perhaps similar to a discoid meniscus in the knee where the meniscus is mis-shaped causing the knee to dislocate at end range extension or maybe she was born without them. And it is around the range where the clavicle starts to rotate upward.

Congenital abnormality?  If it has always done it and does not cause any pain, but when you keep it from happening there is pain, I'm not sure there is anything to do about it.  I do wonder down the road what her experience will be with these joints.
