We are in the process of updating our out-patient forms and I am need some assistance. If you use an intake questionaire, what information do you get regarding medical history, etc? Are there examples of these forms available. Thanks for your input.

You may want to use the Guide to PT Practice. In it, there is a pretty thorough outpatient intake. You could use that as your template and add or subtract things that fits you best. In the second edition, page 699 is where they start. I just recently updated ours and found that I wanted to add more to the symptoms within the last year section. Also, you may want to (if not already) put in a question regarding falls within the last year, per Medicare requirements. Let me know if you have any trouble finding the reference.
