I am a graduate student currently enrolled in a research analysis class.  I am performing a survey about the viewpoints of school SLPs on inclusion vs. regular/pull out therapy.  I am going to provide the survey below.  Any help and comments would be greatly appreciated.  Thank you for your time.

Name (Optional):                                                       Age:                            Sex:
Name of School:                                                        Total # of students at your school:
Years Practicing in School Setting:                         Certification Year:

What is the number of students on your caseload?

What is the age range of students on your caseload?

Does therapy take place in a speech room?

How long have you provided services in a speech room?

Do you provide services in collaboration with the teacher? ______ If so, for how many years?

How is therapy different in the regular classroom from the speech room?

What are the PROS of speech language pathologist providing services in the regular classroom (inclusion)?

What are the CONS of speech language pathologist providing services in the regular classroom (inclusion)?

Do you feel that students qualifying for speech benefit more from services in the speech room?  Why or why not?

Do you feel because of inclusion, all students receive the necessary speech language services?  Why or why not?

Do you feel inclusion is just as effective for students with syndromes/disorders as it is for other (normal) students?

Any additional comments or concerns:
