Hello everyone,
I am new in USA and have questions, I know everyone must be busy in their life, but please help me out on this.
I finished my Bachelor degree in Physical Therapist in India. I have experience of working in hospital for 2 years in India. I came to US on right of my US citizen wife. I am in Illinois and I want to apply for the license, so I need to get my credential evaluated by FCCPT. I am confused that what type of evaluation should I choose, type I or the Educational Credentials Review. I am not sure if I need Healthcare Worker Certificate. What is Healthcare Worker Certificate anyone. I know it is kind of long, but I had to explain.

Thank you for your help
