I hope someone here can assist me. Here are the details , a 22 year old thin male involved in MVA ,rear ended . Injuries include lumbar herniated disc ,requiring sugery , neck pain, whip lash ,shoulder pain , surgery suggested for herniated disc ,but not yet  undertaken , sacroiliac joint pain complaints, not  diagmosed officially , but complaints of pain there, MRI of TMJ shows moderate Tempomandibular dysfunction bilaterally . more severe on Lt side with effusion fluid anterior and posterior of each meniscus of the jaw joint with bursae ( fluid of the pytergoid muscle ( jaw) sever grinding joint noise , specialist suggests arthoscopic surgery bilaterally , yet not yet undergone iether. Many injuires m however there is a foot injury that is mysterious and need some input . The accident victim was wearing steel toed boots and when his vehicle was hit fron behind he acted with slamming his left foot on brake and when that wasn;t stopping his vehicle from going into intersection and slamed his right foot on top of left to help the vehicle brake ,there is no swelling now , the accident is 2 years plus past ,but the person has had toothache like pain in left foot across the top of all toes ,and there is a very strange crepitus that NEVER bates across each of the toes , this does not occur on the right foot . The left foot had an MRI and what was found was a stress fracture of the 2nd metatarsel , however the patient is in alot of pain , toochache like that they feel the need to constantly pop those toes across the op and again the noise is unbelievable ! Has anyone seen anything similar , orthapedic doctor at a loss too , patient does walk and has mobility ,yet the pain is constant ache and has to pop pop pop  the joints at the top of the toes every chance given .


Sorry to be so long ,just wanted to give as much detail as can remember.

 I wanted to add , that there was an MRI long post MVA , atleast 1 year after MVA and the finding was only a stress fracture of 2nd metatarsal .In reseraching I am wondering what the chances are there was some possible underlying
Lisfranc Injury or ligament damage ?

Sounds like a foot mechanical restriction of at least the cuneiforms. "Handbook of Osteopathis Technique", by Laurie Hartman, has a section on foot techniques which shows how this is done. I would also suspect other mechanical restrictons, especially the cuboid. Phil Greenman's rule was "if anything is stuck in the foot, the cuboid is also stuck". See Jim Meadow's online site for the talo-crual joint technique and in Hatrman, the tibio-talus technique. Any and all of these, and more may be going on. I see this in MVA's all the time.
