 I have run for 18 years.  My CMP symptoms in my left knee came on suddenly approx 2 years ago (no pain...just woke up with decreased knee flexion).  I specialize in acute care, yet I am attempting to treat myself for my CMP (Grade III; 3 x 2 cm defect on lateral patella).  I had a chondroplasty and LOA (adhesions were superior and lateral) last year.  With little relief, I opted to try Synvisc injections.  My eccentric control of my quads is now even worse, and I fear worsening of the defect.  My surgeon says that my defect is too diffuse for any of the cartilage plug surgeries.  I have now decided to see an out-patient PT, yet my appointment is not for another month.  I need some advice and some hope that my function can improve.

I would say see one of the ortho PTs.  You probably need your feet looked at, ITB tested for tightness, same for the quads.  Lateral patellar retinaculum probably needs to be mobilized.  My opinion, if you have chronic-ish patello-femoral syndrome you can't fix it by treating the knee.  It is almost always the foot and the butt's fault.  They fail to do what they are supposed to do and the patella gets beaten up.

I agree with Tom.  We, overall, have treated knees way too much just to find the patients didn't get better!  Check above and below.  What's the reason for such a delay in starting OP PT?

I think the delay was multifactoral.  My surgery was approx 1 year after the onset of symptoms.  I got pregnant shortly thereafter.  I had hope that the continued pain would get better after I lost the pregnancy weight; therefore, I waited those 10 months.  The pain/poor eccentric control continued, and I only now feel comfortable leaving my baby with someone else. 
I also wanted to make sure that I found a therapist who had great outcomes, and I only recently found her.
I guess I was in denial.  I just kept hoping that something I tried would work.
I may have neglected the butt, yet I did go and get custom orthotics as I do overpronate some. 
I was able to get in to see the out-patient PT on a cancellation, and she feels they are not sufficient (not full length, made of plastic, etc.).  I was casted in sub-talar neutral, yet this technique may have not been the best.
There are many things I don't understand about my issues.  I don't understand why the CMP onset was sudden, not gradual.  I don't understand why I cannot maintain eccentric quad control after obtaining it.  I suppose your arches can fall over time, yet I have run over 18 years without orthotics with absolutely no problems.
I have learned that I do specialize in acute care, and I do need the help of an out-patient therapist.  I am praying for some good outcomes as my everyday function is very limited from my baseline.  Thanks for your input.
