Recently discussed in the APTA Peds PT Discussion Listserve was the research and ideas of contraint induced (forced use) therapy for hemiplegia. The Taub et al. studies, Ostendorf and Wolf, Wolf et al., Blanton and Wolf, Leipert and Taub et al., etc. are some of the names on the published literature for adult constraint induced therapy. Yet to be published is the work of a PT regarding her small study of 6 children with hemiplegic CP and constraint therapy. Does anyone have experience in trying constraint therapy with a child that has hemiplegia as a result of a recent traumatic event (brain tumor removal left lateral ventricle)? The child I am working with has residual foot drop as the primary lower extremity dysfunction. Does anybody have ideas how to take the ideas of constraint therapy for the upper extremity and use constraint therapy of some type on the lower extremity as well? I was thinking of trying a knee immobilizer on the affected limb to force more active dorsiflexion for brief periods of time during the day. Any ideas are welcome. Thanks, Bob Wiederien
