A Knife.
As unappealing as surgery is, the orthopods in my area have had great success in treating moderate to severe CTS.
The other great modality is prevention. Good ergonomics and job selection. Just as people who don't like snow should not move to Alaska, People who's bodies have small carpal tunnels should avoid jobs / activities which require repetive hand movements.
In mild cases, modalities which reduce inflammation- rest, bracing, soft tissue mobilization, ice, heat, ultrasound - are of some benefit.

don't forget the chance of double crush injury screening out the cervical is important

anpt2003,
i've had great success treating pts w/ IOP and dexamethasone. in combination w/ splinting, specific hep and nerve/tendon glide exercises we've been able to help a number of pts avoid the knife.

I agree with the previous:
Conservative regimens fo Ionto, splinting (in neutral) and tendon/nerve gliding have proven effective.

Personally i've found STM, icing, ultrasound to be useless is about 50% of the cases I hace treated. The best results seem to come thru splinting, stretching exercises, and prevention of repetitive movements.....
