I am treating an 82 yo male s/p MVA(11weeks) with TBI, as well as global hypoxia. PLF was independent, active--drove, still worked his farm, had had laminectomy ~2 yrs ago and total shoulder replacement last year, no other health problems. He was comatosed x4 weeks, has been off vent for 3weeks, had two weeks of rehab at the hospital and was recently transfered to my NH. He is actually quite functional, cogntive level VI, transfers with min to contact guard assist, amb with min assist with a walker. Problem is his COG is posterior in stand. I have worked with him (with and without mirror)on the ball in sitting, on the mat in prone thru tall kneel--all with relative good postural alignment and righting reactions. However, once we stand everything is shifted backwards. Does anyone have any suggestions on how to correct this?

Get something small, smooth and hard like a small pebble or dried bean. Tape it to the sole of his foot in the center of the area just below the fat pad at the met heads. Have him practice wt shifting forward onto the pebbles to find his balance point. Talk him thru it. You can eventually get the same results by having him "remember the pebbles". Sometimes that little trick works miracles.
