Well I am posting here because I am hoping to get a few opinions and maybe a solution to a nagging issue I have been having.  First off I found this forum a few years ago when I was a personal trainer, but I had to sign up again.

Injury....2 years ago I was playing baseball and slide into home plate, felt fine all night but the next day I had a tightness between my shoulder blades.  I also had a slight burning sensation located around my left shoulder blade.  Since this injury I have had repeat injuries upper and lower back, and also a shoulder injury.....The burning feeling has gone away but the tightness is constant between my shoulder blades.

I have gone to multiple chiropractors, family doctor and physical therapy.  The chiros did adjustments, which helped the lower back but never fixed the mid back.  It always feels as if its out of adjustment...like it "needs" to move.  I was also told its due to my active life, I am 32 now......Before this I was VERY active and healthy.  The family doctor advised me to go to the chiro....I was feeling pretty good, so I went and played football  Sorry I know, but i was feeling good.....I dove and my shoulder now hurts with internal rotation "as in a boxing punch"

Sorry this was a mouthful...please ask some questions.  Its my feeling that something is wrong and like a twist in a rope its just moving from lower back to midback.....causing issues depending on where is is located.

You said you went to P.T. but you didn't say if it helped (which I'm guessing it didn't or else you wouldn't post here) and what they did for you or thought.  Did you see a board certified P.T. in Orthopaedics or Sports?  There is lots of assessment involved in treating something like this and I believe you will have a hard time getting someone to help you online for this reason.  I would look into seeing a board certified P.T. or one who is reputable in your location.

Yes I did go and they moved me through a range of motion and determined that it was weak shoulder stabilizes, simply based on the fact that it didnt hurt.  These motions were very slow and controlled, not fast which is a primary cause of pain....Remember I said that I was very active and fit before these injuries.  I did rotation exercises but nothing that really was similar to the motion that bothers it...I can do bench press although have not pushed it, I am pretty sure pull ups would hurt, Fast movements are much more likely to cause pain, ie playing basketball and reaching to block a shot for example or bear crawling fast...Or as in my first post a boxing punch (last portion is internal rotation) This punching is only with upward not straight out.....I appreciate your respoonse and agree that seeing someone might be a good idea, its just that I am a volunteer fire fighter as my primary job (doesn't pay much) I have no insurance and I have already spent 1000+ with ciro, 350 with physical therapist and 300 with family doctor....My pocket is becoming empty fast and am afriad of just being passed around until I am broke and broken:D

What hurts with boxing punch and other "quick movements"?  Specifically, not just shoulder or back.  Where on your shoulder/back -front, back, top, inside, etc?  Do you have catching or "popping" in you shoulder with ROM?  Do neck movements have any effect on your symptoms?  Anything that makes your symptoms better or less?  Did you folow through with strengthening you shoulder stabilizers (remember, muscle hypertrophy takes upto 8 weeks of working at >= 60% of your maximum voluntary contraction force)?

Interesting presentation thus far.  When you say rotation are you talking trunk or shoulder?  Also, I certainly understand if you do not have insurance and finances are tough to come by.  But just remember that if you continue to have pain and worsen things then this may affect your job and in hindsight P.T.may have been worth all the money.

I would advise that you get your neck evaluated by a good PT.  Weakness in scapular stabilizers does not cause scapular pain, unless it is profound, as in neurological. Also, You do NOT hurt because something it out of alignment.  This is another myth.  You slid into home, your head and neck jarred around like a bobble head doll, and this is where your treatment needs to be focused until proven otherwise.  You have spent a lot of time and money addressing things in and around the scapula without great benefit, so the need to concentrate elsewhere is evident.

Thank you all for your replies I will try to answer all the questions as orderly as possible.  The pain with the quick movements is in the shoulder....specifically it feels as if its the lateral aspect of the deltoid.  I do not have any poping or catching and I would say that neck movement doesn't effect the pain.  When I lay on my back it feels very weak, when placed overhead...Infact If I lay on my back and raise it straight up and over head (towards the floor) I can cause "pain" and "tightness."  I did perform my exercises and didn't feel that they were even that challenging....except for the balancing:D They had me do alot of balance and catch a ball or one leg standing while moving exercises....along with the "baseball" throw tubing rotation.

The internal rotation I was refering to was the shoulder.....When boxing the last few inches of a punch include a internal rotation in a "pop" fashion.....I wasn't clear before because I was trying not to be long winded.  I can punch a heavy bag no problem...Its when I do conditioning drills such as "reach for the gold".  In this drill you punch up over your head for 3 minutes.  The hands begin palms towards face and arm extends upward while hand rotates to palm away from the body position at full extension. Agreed with pt being valueable, I noticed that if "adjustments" are going to have effect it is within the first few sessions and certain types of feelings....I wont try to explain the difference

Ok.....regarding the scapular pain.  I do not have pain anymore in that area....although my back always feels "out of alignment"  I feel as if tension moves up and down my back....sometimes my lower back has knots and others I have alot of tightness between my shoulder blades.  If I bend over I feel a "poping" and adjusting as if things are shifting between my shoulder blades.   I did have burning sensations on my left shoulder, which is the one that hurts me originally but there was a year between these feelings and any problems with the shoulder. If I had said the pain was because something was out of alignment I am sorry cause that would not be correct.  What I had said was that I "feel" as if maybe something is out of alignment which is changing how "all" my muscles are alignment thus messinging up the mechanics of my body.  This would increase the chances of muscular injury.  I would think that the chiropractors would have been addressing the neck area, which was supposibly better then normal reagarding the natural curvature of my spine.


Slide into home plate (2 years or more ago)
   burning sensation behind shoulder blade and left shoulder
   tightness between shoulder blades
  
Massage and Chiro appointments
    burning sensation slowly fades and tightness lesses but doesnt go away....poping and adjusting       between shoulder blades when I bend forward and touch toes
    start having issues in lowerback.......determine to be slipped disk, ciro fixes within 2 appointments, now just occausionally feel knots in lower back area

Feeling good overall, learned to live with "tightness and poping" between shoulder blades.  I was told its just that the ligaments might have been strained when I hurt myself and they need to "tighten" over time again
    I begin running and doing light conditioning workouts....feeling good
    play football, dive....next day shoulder is very sore and hasnt healed completely in 5 months of very light activity (try to protect it as much as possible)
          I have done lots of massage (earned in trade) and did pt exercises under supervision and in own workout...despite them not feeling like the really challenged the area of concern

Sorry this rambled off.....Its just there is so much information that might be a key for one of you to understand my condition.  Pain is such a relative thing, my grandfather "hurt" his shoulder when younger, and it was just a minor thing....now we find out he completely tore a rotator and as his major muscle atrophy he is loosing alot of his shoulder motion because they compensated for the lack for stabilizer muscles
