Achilles repair 11 weeks ago after tearing playing basketball, male mid 30's and healthy as a horse. Doctor has let him advance quickly due to age/clean tear and repair, out of a boot in 4 weeks and told patient to use Crocs instead of sneakers due to pressure on posterior ankle. Has a guideline of running/jumping at 6 months, and he's about to go AWOL if he can't leave the treadmill for the elliptical. He has full active/passive plantarflexion, 75% single leg calf raise (allowed by MD). Any thoughts of why he can't be on the elliptical? Thanks

Does your ellipital have different settings? such as incline, etc? Im asking b/c I recently treated a 55 y.o runner with an achilles tendon tear. after 3 weeks of treatment, stuck him on the elliptical. He did well. Kept him on a low incline for 5 min and increased as appropriate.

Do you know about stresses on the Achilles using the elliptical? The treadmill and walking have histories of being successful during Achilles repair rehab. I do not know about the elliptical machines being used at 11 weeks post repair. Is the ellipitical machine similar to walking? Similar enough that you are confident it does not over stress the repair? I would probably spend time talking the patient into following an established guideline and time frame for the rehab program. Encourage the patient to understand the rehab progressions with Achilles repair should be slow and graded. Be sure to work on plantar flexion strength at end ranges and be cautious with stretching.

I agree that there has not been much to my knowledge scientifically studied regarding their use and the Achilles. I was wondering if there were any individuals who had such knowledge. I also agree with you that without specifics that he is stuck with the treadmill and walking. Thank you for your time and input

polishPT - I am curious about how your patient is doing. Did he jump ship to go on his own? Is he following therapeutic advice?

Is there anyway you could drop the elliptical in a pool? Kidding. I don't see any problem with the elliptical as long as he doesn't go ape on it. Just progress like anything else slowly and steadily. Good luck.

LOL Rod! I would do the same, just monitor and make sure any lingering pain/discomfort drops down to the same prior level of discomfort/pain during walking (a quick functional baseline) about 15-20 minutes after finishing that specific bout of exercise. We treat a ton of achilles (triathlete central over here...it's nuts a month before a race and we get flooded with about 20 cases of minor-moderate achilles dysfunction/tendonosis)
