I was wondering for those of you who see a lot of ACL's on a pre-op basis. How much closed chain work do you emphasize. I know the main goals are range and reduce swelling,etc but primarily I was wondering once ROM and swelling have improved how much you push closed chain work knowing they don't have great stability. Thanks in advance for any comments. Adam

honestly depends on several factors: age, PLOF, co-existing damage inside the knee, etc. If pt can tolerate partial range squats, leg press, etc, go for it. Decreased strain on the ACL in closed chain anyway as you can focus on co-contractions. go to pubmed, copy and paste the info below for a good article with strains using leg press. Med Sci Sports Exerc. 2001 Sep;33(9):1552-66.

As mentioned, the amount of closed chain work will depend on the other structural damage within the knee, meniscus tears, OCD, Bone bruise etc. Based on pain, if they can tolerate weight bearing squat, lunge work then I beleive it has tremendous value to introduce those exericses and teach proper mechanics before surgery. Teaching them after surgery can often be more difficult due to post surgical pain, fear, additional weakness and balance deficits. Gary
